COVID-19 Vaccination
FDU is working with regional health facilities (Holy Name Medical Center in Teaneck, N.J., and Atlantic Health System in Morristown, N.J.) to facilitate vaccinations for students, faculty, and staff. You can register directly for either option. Go to the Holy Name Vaccine Registration Portal and, in the category of “Eligibility Information,” faculty, staff, and students should select “Educator/Childcare,” and then list Fairleigh Dickinson University as the organization. For Atlantic Health System, go to COVID-19 Vaccine – FDU – Atlantic Health.
When you receive your final vaccine injection (second shot of Pfizer or Moderna, only shot of J&J) please log in to the Medicat Patient Portal and upload your vaccination record.
COVID-19 Operations Notice
For the health and safety of everyone at FDU, face masks must be worn in our office at all times. All students will require an appointment to be seen by our staff. Please call 201-692-2437 to schedule one. You will not be seen without an appointment.
Please expect to answer a few screening questions in order to determine the type of appointment that is needed.
Welcome
Student Health Services Metropolitan Campus (SHSMetro) is a health center staffed by a Nurse Practitioner and Registered Nurses who are available for the health care needs of the FDU students. SHSMetro provides direct care, individual health counseling, and health teaching. Small group workshops occur throughout the year on common health interests of the students.
All registered students can receive health care services, including students who have waived the University’s United Healthcare Insurance. Our goal is to provide health and support services that will enable students to perform at the highest level of their ability with academic success as the goal.
Medical Entrance Requirements
As all institutions of higher education have an obligation to ensure the public health of their students, we collect and review medical and immunization records for all matriculated FDU students. Please see the Immunization Policy for Students for more information.
All medical forms must be submitted through Medicat. The Medicat patient portal can be accessed by using the same username and password that you use to access your FDU student account. At Medicat you will be given instructions on how to complete your medical records. If you are having technical difficulties accessing Medicat please contact the Help Desk at 973-443-8822.
Medical records are due on the dates as seen below:
- Fall Semester: July 15
- Spring Semester: December 1
- Summer Semesters: April 15
