Request for Documents

To request official documents from Fairleigh Dickinson University (FDU) Vancouver, students need to submit the Document Request form to the Office of Enrollment Services. 

What kind of documents can I request with the document request form? 

  1. Confirmation of Enrollment
    • Students may require a Confirmation of Enrollment Letter (COE) for immigration purposes, travel, or for an employer. 
  2. Syllabus
    • A syllabus is an outline of the subjects in the course of study. 
  3. Tax forms
    • Tax forms are issued annually, recognizing education-related activity within the tax (calendar) year. 
    • NOTE: This option only applies if tax forms are not found in Self-Service. 

Students who do not have access to their FDU NetID should fill out the Public Document Request form to request official documents.

Students must be in good financial standing to receive official documents. FDU shall have the right to withhold enrollment letters, official transcripts, diplomas, and any Certificate of Completion, to the fullest extent permissible by law.
